Doripenem

Doripenem is an antibiotic of the Carbapenem class. It is a beta-lactam antibiotic capable of killing Pseudomonas aeruginosa. Doripenem is used to treat bacterial infections namely complex abdominal infections, pneumonia contracted in hospital, and complicated infections of the urinary tract including sepsis in the kidney infection. It has greater stability in aqueous solution compared to other Carbapenem class antibiotics. Doripenem is administered as an infusion over 4 hours or more, which is an advantage when treating difficult-to-treat infections. It also has a lower risk associated with inducing seizures than other Carbapenem. Doripenem is sold under the brand names Doribax and Finibax amongst others. Shionogi Co. of Japan launched it under the brand name in 2005 and is marketed outside Japan by Johnson & Johnson. Doripenem was approved by the USFDA in 2007, to be sold as Doribax. It is the fourth carbapenem class drug to get approval in the United States. USFDA later revised the Doripenem label in 2014 to include a warning against use in ventilator-associated pneumonia while reiterating its safety and efficacy for its approved indications.

Forms and Dosage

Doripenem is sold as a powder for injection in a 500 mg vial. In the treatment of Complicated Intra- abdominal Infection, the prescribed adult dosage is 500 mg IV infusion over 1 hour, given every 8hr for 5-14 days. if there is an improvement in the condition, the doctor may switch to PO treatment. In the treatment of Complicated UTI, the standard prescribed dose is 500 mg IV infusion over 1 hour given every 8 hours for 10 days, which can be extended up to 14 days if needed. In the treatment of Bronchopulmonary infection in patients with cystic fibrosis and who are colonized with Pseudomonas aeruginosa or Burkholderia cepacia.
