Aprotinin is a serine protease inhibitor used in high-risk patients undergoing cardiopulmonary bypass for coronary artery bypass graft surgery to lower the risk of perioperative blood loss and the requirement for blood transfusions. Aprotinin (Trasylol, formerly Bayer and now Nordic Group pharmaceuticals) is a tiny protein bovine pancreatic trypsin inhibitor (BPTI), also known as basic trypsin inhibitor of bovine pancreas, which is Trypsin and related proteolytic enzymes are inhibited by an antifibrinolytic compound. Aprotinin was marketed as Trasylol, a medicine that was injected to minimize bleeding during complicated surgeries such as heart and liver operations. Its main action is to slow down fibrinolysis, the process that causes blood clots to dissolve. Its use was intended to reduce the need for blood transfusions during operation, as well as end-organ damage caused by hypotension (low blood pressure) caused by significant blood loss. After investigations revealed that using the medicine raised the risk of complications or death, the drug was temporarily removed worldwide in 2007. This was verified by follow-up studies. Trasylol sales were halted in May 2008, except for very limited research use. The European Medicines Agency's (EMA) scientific committee reversed its previous position on aprotinin in February 2012, recommending that the suspension be lifted.
